
U.S. citizens must have a valid passport to travel from Uruguay to the United States.
It is obligatory to bring copies of all required documentation for your passport application including parents’ passports/ID’s if applicable.
If you don’t have the copies at your appointment, you will be asked to leave the Embassy to obtain copies and return before we can process your application.

Applying for a Passport for the First Time as an Adult
If you are applying for a U.S. passport for the first time and you are over 16, you must come in person to the American Citizen Services Unit of the U.S. Embassy in Montevideo after scheduling an appointment through our Online Appointment Scheduling System.
Please keep a copy of your appointment’s confirmation page and bring it with you to the appointment.
Documents you need to bring to your appointment
- Form DS-11 (completed, but not signed);
- (1) Passport-size photo (2” x 2”);
- ORIGINAL U.S. birth certificate; or
- ORIGINAL U.S. naturalization certificate; or
- ORIGINAL U.S. certificate of citizenship;
- Previous U.S. passport (if any);
- Valid photo ID (American or foreign);
- Money to pay the passport application fee (can be paid cash in dollars or pesos or with an international credit card). We do not accept checks or debit cards.

Important information
- 16- or 17-year-old applicants must come to their appointment with one of their parents (the father/mother coming to the appointment must bring a valid photo ID).
- If the most recent passport was lost or stolen, please complete and sign form DS-64 and file a police report. Bring both documents to your appointment.

Picking up your passport: Once your passport application is submitted, the passport will be ready for pick-up within approximately 5 weeks. As soon as we have received your passport in Montevideo, we will send you an e-mail stating how and when you or a third party can pick it up. For pick-ups, the receipt for the service must be shown.

Important information:

- If one of the parents cannot be present at the time of presenting the form, he/she must complete Form DS-3053 and sign it before a Notary Public. A photocopy of the signing parent’s valid ID must be attached to this form. Any U.S. Embassy can notarize this document free of charge, so we recommend doing this at the Consular Section of the U.S. Embassy. Please keep in mind that you need a copy of the form for each passport applicant.
Extraordinary circumstances or special family situations:
If one of the parents cannot locate the other one due to extraordinary circumstances or special family situations, the parent appearing to present the form must complete Form DS-5525.
We would like to emphasize that the use of this form is very exceptional and it should only be used in cases when the other parent cannot be located, or if any court or legal documents establish that the appearing parent is the sole guardian of the child.

Important information:
You can use form DS-82 if your most recent U.S. passport:
- Is in good condition and can be presented with your application.
- Was issued when you were 16 or older.
- Was issued within the last 15 years.
If you answer NO to any of the above requirements, then you are not eligible to use form DS-82 and must use form DS-11.

Requesting Emergency Passports
In the event of an emergency (e.g., stolen passport, limited travel time, etc.), the American Citizen Services Unit can issue temporary passports on the same day you apply.

Correcting Mistakes or Changing your Name
If your passport was issued with a mistake in your biographical data page, the American Citizen Services Unit (ACS) at the U.S. Embassy in Montevideo will correct the error.
If your name changed due to marriage or by court order, you must come in person to the ACS Unit.
Important: In order to be able to correct or update your passport data, you must schedule an appointment through our Online Appointment Scheduling System. Please keep a copy of your appointment’s confirmation page and bring it with you to the appointment.
Documents you need to bring to your appointment
- Form DS-5504 (completed and signed);
- (1) Passport-size photo (2” x 2”);
- Previous U.S. passport;
- Valid photo ID (American or foreign);
- If you need to correct your name or other biographical information, you must bring your birth certificate or previous passport with the correct information. If your name has changed due to marriage, you must bring an original marriage certificate. If your name has changed due to a court order, you must bring the original court documents indicating the name change.
